Which way do I go ?

I am planning to go to Peru in 2019 and am making preliminary plans now. Since most airlines make the connection in Lima, I figured I would spend 2 days in Lima: arriving early day 1 then having the next day and the thrid day flying from Lima to Cuzco. My question is this; Do I do Lima first then the Inka trail or the trail first then the 2 days in Lima?

If you are considering a trip that starts in Cuzco (as opposed to Lima), I would spend an extra night or two in Cuzco before your trip starts. You only get 1 night in Cuzco (and one in Ollantaytambo - at a much lower altitude) before starting your trek. There is a lot to see around Cuzco that you won't see on your tour and it will give you time to acclimatize better.

As for your actual question, I prefer to add on days on my own at the end of the trip when I am more familiar with the country. If you can get a connecting flight within a few hours of arrival in Lima, all the better to get to Cuzco. You might be able to add it on to your international flight for less than buying a separate ticket. I managed to do it on points (Aeroplan - Star Alliance) and it didn't cost me any extra. If you do choose to go directly to Cuzco from your international flight and book the ticket separately, make sure that you have travel insurance that will cover you if you miss the connection as neither airline will take responsibility.

As an alternate point of view, I almost always book my vacation to have an extra day in-country before the joining meeting. If I get in on time and without any unforeseen calamities (like my luggage being lost), this extra day can go toward just getting acclimatized and oriented to the area, exploring my surrounds, etc. but it gives me some cushion in case things DON'T go as planned.

I then like to add another day (or two) on the end so I can do last minute souvenir shopping, have a moment to compose myself (shower up, buy replacement items, etc.) and pack my luggage well, etc. If I have any vacation days left over, I then decide on where to apply them -- most likely pre-tour so I have that much more cushion.
