Online medical form
I can't complete the 'online medical form'. It let's me enter the 'booking reference' and 'customer number', but won't go any further after I press submit. What should I do?

Does everyone have to complete this form? Didn't book direct through gap and wasn't aware of it until I saw it on this website!!

Who Should Complete This Form?
All travellers must complete the form. If you indicate that you have a pre-existing medical condition your medical practitioner must complete Section D - Part 2 of the printable medical form. The more information Gap Adventures has, the more we may assist you in the unlikely event of an emergency or need for other medical assistance.
All travellers to Antarctica are required by the International Association of Antarctica Tour Operators (IAATO) to provide insurance and emergency contact information.

I've booked the trip ad a travel agency. Now I wanted to coplete the medical form, but I'm not able to find my customer number. I have a "booking file" on my sevice voucher, but there is no customer number. How can I complete this form?

We have lodged our medical forms by email (documents@gadventures.com), but have yet to receive a response. Should we expect one, or will the response go to our travel agent?
Also, the online medical form forced me to complete a hard copy version because I responded 'yes' to question 5 (allergies/reactions to medications), but the hard copy form did not require me to complete Part D. It would be handy if the online version of the form could be adjusted to allow it to go through even if the answer to question 5 is yes.

Hello,
I've just come across this post. Recently a G Adventures adviser told me I don't have to complete the medical form unless I have a medical condition, on medication or had surgery in the last 5 years. Yet on the website and above it says everyone must complete it. Do I or don't I? Also I think I have a booking number but no customer number as I never received one.. so can't actually get to the medical form anyway.

You can get your customer number from G's customer service, either by phone or e-mail. The forms are mandatory for some trips but not all. Which trip are you going on?

Thanks for that. I'm going on the Coast to Coast - Eastbound trip. Just a bit concerned about the conflicting instructions as I don't want it to jeopardise anything.

Travellers must be able to walk without the aid of another person, climb 3-4 flights of stairs, step on and off small boats, and carry their own luggage at a minimum. Travellers with a pre-existing medical condition are required to complete a short medical questionnaire, which must be signed by their physician. This is to ensure that travellers have the necessary fitness and mobility to comfortably complete their chosen trip.

I've never completed the form and never been asked about it (6 trips). I've looked at the form and although I don't have any of the medical conditions listed or any other medical condition that would affect me on a trip I do take a regular medication (that I could skip for 1 - 3 weeks without any problems) and I was ill about 2 years ago with a "significant illness" from which I fully recovered. To me it is a waste of time and money, as well as the scarce resources of the medical system to have my doctor sign the form. I know I'm not the only one. The way the form is structured, any woman taking birth control pills or someone who broke their arm 4 1/2 years ago and required surgery to set it needs to have the form signed.
I've also never been asked to even look at the form when I've signed up for a trip - I've used 2 different travel agents, done it on-line, done it over the phone and at one of the stores in Canada so obviously its not considered that important unless you have a medical condition that could affect you while on the trip. In that case it is a good idea to fill out the form and check with your doctor before departing. You also need to make your leader aware of your condition at the start of your trip.

I've not heard of a single passenger being turned away for not having the form completed. As stated before, for some of the more difficult trips the form is required. Please contact base camp and they can tell you right away if the form is required for you or your trip.

is there any way to get confirmation that the form was received? I faxed it in Thursday from work and got the confirmation from my fax machine that it went through but since I am going to Antarctica it would be nice to know from G's end that it was indeed received.
