Medical forms - er, help please :S
We're booked on the Absolute Peru for THIS saturday, and only found the medical form information tucked away in the information pack a few days ago.
My husband has very mild asthma - no problems with it, just takes an inhaler occasionally after heavy exercise. I phoned up the medical information line for Gap, and was told that we dont need to get a doctor's signature for this, but should disclose mild asthma. However, the form says if you tick yes to "Asthma effects my everyday activities and/or I use medication or an inhaler regularly" you do need a doctors signature (which we dont have time to get), and I dont see another way to disclose mild asthma on a medical form, without the requirement for a signature.
The phone line also told me I didn't even need to bother filling in the online form at all, but I went ahead and did it because the online travel information told me too - only because I have at some point in my life had mild depression (~5yrs ago), I ticked yes to that, and now it's asking me to do a medical form as well!
If we turn up at the start of the trip with the paper medical forms, disclosing these "pre-existing conditions" without a doctor's signature but a note saying that the phoneline said we didn't need one, are we in any danger of being refused any part of the trip? Otherwise I may have to somehow get tomorrow off work and sit in the doctor's all day until someone can sign these for us.
We're both fit, healthy and active, and none of this has been any problems for our travel insurance.

Thanks for the reply.
Customer services was who I spoke too on the phone, and they said I don't need to get anything signed. But that contradicts the forms and information online, and I dont think a non-recorded phone call is going to help me if the tour guide turns around on saturday and says we can't do the trip.
Do they reply to posts on here at all? or when you say alert them do you mean they'll contact me some other way?

Hi there!
I have reviewed your message and understand your concerns. As long as you are aware of the physical demands of your Absolute Peru trip, and are able to carry your medication with you, mild asthma is not something we need a Doctors signature for. You will not be refused any part of the tour, it is just good information for your tour leader to know (just in case).
Should you have any further concerns, please feel free to email me at customerservice@gap.ca
We hope you are looking forward to your Peru adventure!!
Kind Regards,
Brock
