If you call base camp they should be able to tell you if the information was received and appended to your file. Let me know if you run into any troubles.

Hi,
I'm booked on to the "Thailand on a Shoestring", "San Francisco to LA", and "Mayan Encounter" tours this summer.
I booked it through an independent company, and my travel agent said that I don't need to complete the form. I asked her again because it said that everyone needs to complete it, but she just told me that if you needed it, you would have been in touch with them (as they booked it for me) and they hadn't heard anything about it from you.
She also said that everyone else she's booked gadventures tours for, didn't have to complete it.
Is this right?
I don't want to turn up and find out that I can't do the tours.

Sounds about right. They always say so to make sure that you're insured adequately. Just have your documents with you at your initial briefing meeting on arrival so that when your CEO asks for details, you have it there to hand over. They're just covering their backs that's all. Many people also book tours when they're travelling already so its difficult for operators to track this information down so the CEO has to check to.

Please get in touch with out base camp and they can confirm if it's required for you personally.
Every person and trip is different. Even I have to fill one out for my upcoming trip.
What happens if I don't complete this form?
In the event you have made a booking with G Adventures and subsequently are unable or refuse to complete this medical form for any reason by the final payment date as specified in our terms and conditions, G Adventures reserves the right to consider your booking cancelled as of that day and applicable cancellation penalties will apply.
